Schools
Transition Plus High School – Partner since 2013
The Student Based Connections Program serves over 20 students at Transition Plus. An advocate spends 2 days a week onsite to complete intakes, get to know students, and check in. The advocate works closely with school social workers to identify students who can benefit from the program. Students are referred by their teachers to participate with Southside Services.
